Throughout the shift, the Lead Care Manager leads a group of Care Managers / “Designated Care Managers”. The Lead Care Manager ensures that residents in our care neighborhoods get and document quality care and services that are consistent with their individualized Service Plan (ISP) and Sunrise policies.
The Lead Care Manager sets a good example in all aspects of their employment and may work flexible hours, including weekend and holiday duties.
Essential Responsibilities:
Provide supervisory support by recording attendance at the start of each shift, managing call-offs and no-shows, providing open shift messages, and responding to shift requests.
Assist team members in providing Activities of Daily Living (ADL) care and services to residents in accordance with Individualized Service Plans (ISPs).
This may involve bathing, grooming, personal and dental hygiene, bowel and bladder functions, dressing, hair care, escorting to and from meals and activities, and cleaning the residents’ room.
Support team members in encouraging tailored and meaningful resident engagement in life enrichment programs and ADLs.
Assist with food serving in the dining room. Educate team members on hospitality standards during meal service.
Maintain accurate documentation of resident care and services. Throughout the shift, review the paperwork created by the care manager. Identify and bridge documentation gaps by coaching team members.
Maintain compliance with Sunrise’s timekeeping and meal/rest period policies. Monitor team members’ breaks and ensure care managers use the relevant work codes when clocking in and out. Coach team members as needed.
Display and encourage flexibility in work schedules by working holidays, evenings, weekends, and additional shifts as needed.
Prepare for the shift by collecting resident group assignments before the care managers arrive. Print and distribute daily assignment sheets to the appropriate team members.
Participate in crossover meetings by exchanging and receiving relevant information with / from other Lead Care Managers.
Carry a resident group assignment on overnight shifts and pick up group assignments in case of an unfulfilled call off on day and evening shifts.
Other responsibilities.
Cross-trained and accredited (including certifications where required by the state) to safely give drugs if needed during high volume or unexpected absence.
Lead by example by clocking in and out and taking adequate food and rest intervals.
Participate in the group interview process, which includes a job preview video and neighborhood tour.
Support the onboarding of new team members, including shadowing and competence demonstration.
Maintain a safe and secure environment for all personnel, residents, and guests by following set safety standards and actively supporting safety practices.
Report incidents and complete the necessary papers quickly.
Communicate any seen or suspected resident change of condition to the department care coordinator; help care coordinators, as needed, in developing and/or updating ISPs and communicating with residents and families.
Prevent and manage conflict by working together to find a solution. Communicate problems to the department’s care coordinator when necessary.
Contribute to the care manager’s annual performance assessment by offering input to the department care coordinator.
Create a pleasant, professional, and friendly image through your actions, words, and attire.
Qualifications:
In states where suitable, must maintain relevant certificates.
Must be at least 18 years old.
Medication Management Certificate is necessary; continuing training and re-education as per Sunrise and state regulations.
Previous experience working with elderly and a willingness to serve and care for them.
